It contained the Black Diamond District, Clinch Mountain District, Cumberland District, and Lonesome Pine District. State tournament placement

Teams from Region D finished second in the Class A state tournament in 2010 (Honaker) and 2012 (Honaker).

Notes

J.J. Kelly won the Region D tournament in 2005 and finished second in 2003, representing Region D at the state tournament in both years. The school has since closed, with its attendance area now part of Central (Wise).

Saint Paul won the Region D tournament in 1998 and 1999, representing Region D at the state tournament in both years. The school has since closed, with its attendance area now part of Eastside.
