Prior to the 2014 VHSL Realignment, VHSL Region D was a VHSL region in class A, consisting of schools with less than 700 students lying in the Southwest Virginia panhandle. It contained the Black Diamond District, Clinch Mountain District, Cumberland District, and Lonesome Pine District. Like all VHSL regions, it sent two teams to the VHSL Scholastic Bowl state championship tournament for the sixteen-year duration of the original Scholastic Bowl tournament structure.
With the 2018 VHSL Realignment, former Group A Region D teams will be moved from their respective regions (1A West and 2A West, both of which drew from much larger geographical areas than the old Region D) into two new "Region Ds," Region 1D and Region 2D.

State tournament placement
Teams from Region D finished second in the Class A state tournament in 2010 (Honaker), 2012 (Honaker), and 2013 (Honaker).
In 2008, Lee County finished third in the Class A state tournament, notably defeating George Mason High School and Rappahannock County before falling to eventual runner-up West Point (VA).
