It contains the Black Diamond District, Cumberland District, and the Hogoheegee District. With 15 member schools, it is the 2nd-largest Region in Class 1.

Region 1D (along with Region 2D) is essentially a bifurcated half of the former VHSL Region D, which from 1998-2013 included schools that are now classified in VHSL's class 1 and 2 (under the combined "Group A" name, and known as 1A and 2A respectively from 2014-2017). However, a noted change is the entry of the Hogoheegee District (or 'Hogo'), which was formerly a part of Region C. In 2018, Grundy entered 1A from 2A with the creation of Region 1D due to a drop in enrollment, winning the first Region 1D Tournament. From 2014-2017, they remained a member of the Black Diamond District, but district matches did not factor into Regional qualification.

Honaker is the only team in 1D to have appeared at the Regional level every year since Scholastic Bowl's creation in 1998; they are also the only team from 1D to have won a state runner-up or championship (of which they've done both 3 times each); the only other teams in 1D to make a state tournament appearance this decade are J.I. Burton (Norton, VA), Grundy (as a member of 2A), and Chilhowie, who have each appeared at the state level once.
