We've done this at VETO, when there has been a mirror and the two
winners play each other in the "Trans-Canada Championship."  

Perhaps Peter McCorquodale might be able to explain it better, but as
far as I remember, we just set up a three-way call between my house,
the room at Simon Fraser, and Brian Hight in Texas, who volunteered to
moderate.  At my house, we pressed different buttons on separate
phones, while in Simon Fraser I believe they had a speakerphone and
used their regular buzzers.

It doesn't actually allow a lockout system, unfortunately, but the
"buzzers" were recognizable different enough that it wasn't a problem,
at least when I played.
