Prior to the 2014 VHSL Realignment, VHSL Region IV was a VHSL region in class AA, consisting of schools with 700 to 1500 students lying from Roanoke westward. It contained the Piedmont District, River Ridge District, and Southwest District. Like all VHSL regions, it sent two teams to the VHSL Scholastic Bowl state championship tournament for the sixteen-year duration of the original Scholastic Bowl tournament structure.

State tournament placement

Teams from Region IV won the Class AA state tournament in 2010 (Blacksburg), 2011 (Christiansburg), and 2012 (Cave Spring).

Teams from Region IV finished second in the Class AA state tournament in 2000 (Graham), 2008 (Cave Spring), and 2012 (Christiansburg).

Notes

Graham won the Region IV tournament in 1998, 1999, 2000, and 2001, and finished second in 2007, representing the region at the state tournament all five times, before moving down to Class A and joining Region D.

Fieldale-Colinsville finished second in the Region IV tournament in 2000 and won the tournament in 2002, representing the region at the state tournament in both years. The school has since closed, with its attendance area now part of Bassett.

Gate City finished second in the Region IV tournament in 2001, and represented the region at the state tournament, before moving down to Class A and joining Region D.
