The Misconduct Reporting Form is an anonymous online form for reporting instances of inappropriate behavior observed at quizbowl events or as part of the quizbowl community.[1]
The form can be accessed here: link
PACE, NAQT, ACF, and IAC want everyone to feel safe and welcome at our tournaments. If you have concerns about inappropriate behavior from a player, a coach, a parent, or a member of the tournament staff, we would like to know about them. This form can be used by anyone, whether a member of the quizbowl community or not, to report an incident of misconduct.

History
The creation of the form was led by Alex Damisch and was part of the response of the quizbowl community to a number of revelations of misconduct in quizbowl in 2018. The form includes representatives from all of the major national quizbowl organizations (as of 2020) to ensure that such information is shared.
